
Mobile Testing Appointments - Augustine Park

Description
Please arrive at the testing location 5 minutes prior to your appointment time. Please bring your ID and insurance documents with you. The testing process should take approximately 5 minutes and will be slightly uncomfortable, but not painful. Results are usually available 24 hours after testing is complete. If you test positive you will be contacted by a member for the Milpitas Fire Department and be given resources and information regarding how to self-isolate and referrals to County and City programs that might assist you. If you test negative you will receive notification via US Mail within one week.
To complete online registration, you must enter one of the following:
Driver's License Number
Social Security Number
State ID Number
If you do not have or are not able to locate an ID number, you can get tested by Santa Clara County Public Health at the Milpitas Library Garage, 161 N. Main St., seven days a week. For information on this testing site visit sccfreetest.org?. or call 211!
