The Waterford School District, its employees and board members, either employed or voluntary, assume no responsibility whatever for any injury by the participant in any Community Education activity. The participant/guardian shall indemnify and hold harmless the Waterford School District, its employees and board members, either employed or voluntary against any and all loss, medical, cost (including attorney fees), damages expense and liability (including statutory liability and liability under workers compensation laws) in connection for claims for damages as a result of injury or death of the participant in use of the facility and which arise from or in any manner of any act or neglect of other individuals utilizing the facilities. Further, to the best of my ability, I hereby certify that I/my child is in good health and physically able to participate in this activity and acknowledge the above medical conditions. I understand that all entries are accepted with the understanding that I/my child agree to abide by the rules and regulations of the department.
