Form to apply for a Replacement Annual Sticker

Please use this form as an application for a Replacement Annual Sticker or update in license plate information. A change will only be issued at no cost to the applicant if the original receipt number of the purchase can be supplied. This receipt number can be found via your account and/or a receipt was sent via email from the original purchase. If you purchased your sticker in person at the Waukesha County Parks Admin Office, Retzer Nature Center, or to resolve a violation, your receipt number would be on the paperwork that you received at time of purchase. Please complete the following application and submit for review.
