RELEASE OF LIABILITY

I give permission for the minor in my custody to participate in the Recreation Division programs and hereby waive, release and discharge any and all claims or rights to claims for damages for death, personal injury or property damage which I may have, or which may hereafter accrue to me, as a result of said minor's participation in Recreation programs. It is further understood and agreed that this waiver, release, and assumption of risks is to be binding on my heirs and assigns.

In the event of sudden illness, accident or injury which may occur while said minor is involved in San Bernardino Parks, Recreation and Community Services Department programs supervised by the Recreation Division or contractors, when neither the parents, guardian or designated family physician can be contacted, I hereby give my consent pursuant to California Civil Code #25.8 for emergency treatment as shall be necessary under the circumstances by any physician, dentist, hospital clinic, nurse or medical care provider licensed under the laws of the State of California. I agree to pay all costs for any such treatment and to indemnify and hold the City of San Bernardino, its employees, officers, agents and elected officials free and harmless for the cost of such treatment and for any medical malpractice arising for which said City of San Bernardino, its employees, officers, agents and elected officials might otherwise be liable.

I hereby grant permission to the City of San Bernardino, its employees, agents, officers, affiliates, elected officials, successors and assigns (herein collectively referred t as 'City'), the right, but not the obligation, to use my name, photograph, video, audtio, likeness, voice, appearance, statements, biographical information, and any other personal characteristics as may be embodied in any visual images and/or audio sounds supplied by me to, or taken of me by, the City. I agree that the City owns the images and/or audio and all rights related to them without limitation. The images and/or audio may be used in any and all manner of media without notifying me, for any purpose consistent with the City's publicity objectives. By signing below, I am at least 18 years old of age and am competent to sign this release. I have read this release before signing, I understand its contents, meaning and impact, and I freely accept the terms.
