General Waiver:

To the fullest extent allowed by law, the undersigned hereby RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE the City, its officers, elected officials, agents, or employees ("Releases") from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, to me (or my child, ward or individual on behalf of whom I am signing)related to participation in any activity offered by the City. I am aware of the risks and hazards connected with participation in the activity. This waiver and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased. In the event of an accident arising out of the activity, I hereby authorize medical assistance to be administered to me or my child.

If purchasing a pass to Los Flores Ranch Park:

I am making this agreement on my own behalf and for the minor(s) whose name(s) appear on my account, in exchange for the opportunity to enter, observe, or use the City's property at Los Flores Ranch Park (the "Property"). I acknowledge portions of the Property were and are used for oil development; otherwise, the Property is undeveloped. The Property contains pipes, wells, sumps, and other features of oil development, as well as unknown and unexplored land features, flora, and fauna. Entering the Property involves risk of bodily injury, death, property damage, or loss due to unseen materials or features in the soil, or gas in the air. I am voluntarily entering, observing, or using the Property knowing the danger involved and hereby agree to accept any and all risks of bodily injury, death, damage, or loss.

Photo Release:

I authorize the City of Santa Maria to take or use photographs, digital images, and audio/video media or other reproductions of me, and/or of my minor child(ren), for any and all purposes without compensation, including publication on social media, television, or other printed or electronic material, and acknowledge same to be the City's property.
