CONSENT TO PARTICIPATE: Yes, I would like to participate in the Whole Child Access Pass program. Tacoma Public Schools has jointly developed an opportunity to provide special opportunities for Tacoma students access to participate in programs and services offered by Metro Parks Tacoma and other non-profit partners. This consent allows for the release of limited student information, to include: STUDENT NAME, SCHOOL NAME, GRADE, PARENT NAME, EMAIL, STUDENT HOME ADDRESS, FREE AND REDUCED LUNCH INFORMATION (if applicable) with Metro Parks Tacoma, in order to provide better fee waiver assistance to participants and the ability to communicate future youth service opportunities to Tacoma Public School families. Additionally, at any time - authorized parents/guardians can opt-out by contacting Metro Parks Tacoma's main headquarters and request to remove their student's name from the program.
