PAYMENT PLAN WAIVER: (only for customers with a payment plan listed above this waiver) By agreeing to set up a payment plan for programs on this receipt, you understand you are agreeing to make payments on time based on the schedule listed above with payment due dates. If payments are not completed by the due dates, you understand you (or your child/family member) will be removed from the program before the start date. For the RLC before and after care programs, this will result in a late payment fee being added of $50.00 and if not paid in full by the end of the month, removal from the program.

Childcare Auto Deduct Payments: Once an automatic payment plan is set up to automatically charge your childcare payment on the 17th of each month, you are agreeing to authorize Howard County Recreation and Parks to automatically charge your credit card or electronic check account listed with this payment plan. I understand this agreement will remain in effect for this entire school year or until I have notified the Department of Recreation and Parks, in writing, of my desire to terminate this agreement, at least (5) five business days prior to the next scheduled payment due date.

Refund Policy is listed in the waiver, brochure, online, and your receipt.

We reserve the right to cancel or postpone any program or activity due to inclement weather or low enrollment. A service fee will be imposed for all returned checks and could result in refusal of future participation. A service fee will be applied to all refunds and withdrawals with the exception of those due to program cancellation or medical/illness. There are no refunds for missed sessions. At a minimum a 20% administrative fee applies to refund requests which includes early termination of memberships; additional fees may be deducted. At least two weeks advance notice is required to avoid additional charges. Summer Camps have a different refund policy and can be found in the current year brochure. See brochure or website for details about all refunds and polices.

This information is provided for families with children enrolled into our Recreational Licensed Childcare programs: Federal Tax ID number 52-6000965. Please keep receipts for tax purposes as stated in the Parent Manual we do not provide tax statements.

Recreation and Parks Waiver: In consideration of my (and/or my childs) participation in this activity, I hereby release and discharge the Organization, and its representatives, successors, and assigns, from any and all liability arising from accident, injury, and illness that I (he/she) may suffer as a result of my (our) participation in this activity. I (we) also will follow the rules and regulations set by the Organization and above named parties. Parent or guardian must sign for anyone age 18 and under. I do hereby grant and give these groups the right to use my or my child(s) photograph or image in conjunction with other persons or objects and presentations, advertising, publicity, and promotion relating thereto.

Completing the registration process for youth sports programs includes an understanding that parents and the athlete have reviewed the information on concussions and sudden cardiac arrest at https://www.howardcountymd.gov/recreation-parks/forms-publications. This is reflected on your receipt and if you have not reviewed this information you must review the information prior to the first session for you to comply with the state law. If you are unable to review this in our activity guide or online/ our registration office can provide you written copies upon request.

By agreeing to the Department’s waiver, you are agreeing that the payment type you are providing is valid and the charge is approved and accepted by the account holder. . Electronic check payments are accepted. Anytime a refund is requested for transactions paid with an ECP, the refund will be via check issued within 10 days by the Howard County Department of Finance. Anytime you need to cancel an ECP payment, please contact the office and our staff can help with the reversal of your payment. If an ECP is revoked by your bank, the office will contact you via email to complete payment with a listed deadline to pay. If the amount remains outstanding, your account will be suspended until payment is received. If the program has not started at the time of the revoked payment and the deadline to pay has pasted, you will be removed from the program and have the option to reenroll based on current space availability.

Medication Form: The Medication Administration Form requires a physician signature for ALL NON-LICENSED camps. The forms are found at https://www.howardcountymd.gov/recreation-parks/forms-publications. Please make copies of the signed form, as it is required at the start of each session of camp.

Information for Summer Camps: For the safety of your child, staff will only release campers to the parent or those individuals listed on the Participant Information Form. Please be prepared to show a government-issued ID when you pick up your child.

Refund Policy for Summer Camps ONLY: We recognize that you may wish to withdraw from a camp for a variety of reasons, but we encourage early decisions so as not to inconvenience other participants or affect our ability to efficiently deliver activities. Customers requesting a refund prior to three weeks before the camp starts will be charged a 20% administrative fee, however if you request the credit to be placed on your Howard County recreation and Parks account for future use, no administrative fee will be charged. All refunds requested within three weeks of the camp starting date will be charged at minimum a 20% administrative fee. Additional fees may be assessed to recover costs (e.g. for shirts, supplies, tickets, equipment etc.) associated with the camp. There are no refunds given for missed camp days.
