PREVENT T2
DIABETES PREVENTION PROGRAM

If you have been diagnosed with pre-diabetes or are concerned that you might be at risk for developing it (defined as having an A1C of 5.7-6.4 percent, or a fasting blood glucose of 100-125 mg/dl) consider joining this year-long program that focuses on diet and exercise to Prevent Type 2 Diabetes.

With the support of a lifestyle coach and your physician, lifestyle changes are made to reduce the risk of developing diabetes. This evidence based diabetes prevention program is supported by the Centers for Disease Control and is free for adults.

  • The core session is weekly for 1 hour and includes education in a group setting for 16 weeks. (Oct. 17- Feb.27)
  • The post core session meets 1 hour monthly for the remaining 6 months.
Program begins: Thursdays, 12:30 p.m.- 1:30 p.m. Oct.17, 2019
Location: Fit Room at Fairgrounds Park (532 N. Cannon Ave.)
