All Participants
Parents MUST fill out a new emergency form. Emergency forms will be available to download towards the conclusion of registration. COMPLETED emergency forms will be submitted to the Rec Center via email to dinubarecreation@yahoo.com or dropped off to the Rec Center. New participants are REQUIRED to provide proof of residency/school verification and birth certificate
Coaches
All coaches are required to complete and submit a new coach's application in order to coach this season and complete a live scan with Dinuba PD. Coach's applications can be picked up at the Rec Center. IF THE REC DOES NOT HAVE A NEW COACH'S APPLICATION ON FILE AND A LIVE SCAN HAS NOT BE COMPLETED, YOU WILL NOT BE ALLOWED TO COACH THIS SEASON.
IN ORDER TO MEET THE REQUIREMENTS OF REGISTRATION, THESE ITEMS MUST BE COMPLETED BY NOVEMBER 8TH AT 12PM. IF THESE REQUIREMENTS ARE NOT MET BY THE DEADLINE PROVIDED, THEN YOUR CHILD'S REGISTRATION IS CONSIDERED INCOMPLETE AND WILL NOT BE ABLE TO PARTICIPATE.
